A Complex Wisdom Tooth and How We Removed It Without Risk to the Nerve

A patient came to us with a complicated lower wisdom tooth. It had only partially erupted, was positioned incorrectly, and its crown was pressing against the root of the adjacent second molar. Because food was constantly getting trapped around both teeth, deep decay developed, pain appeared, and delaying extraction was no longer an option.

The tooth was lying horizontally and had three roots, which is extremely rare. The roots were not only wrapped around the mandibular canal containing the nerve, but were also compressing it, which significantly increased the risks of surgery. In such cases, nerve-related complications are possible, including numbness in the corner of the mouth and chin, or severe pain.

What our oral surgeon did: performed detailed surgical planning based on the tooth anatomy and the exact position of the nerve, used a gentle extraction technique to avoid damaging the mandibular canal, and controlled every step of the procedure to minimize the risk of complications.
