Replacing a Cracked Ceramic Onlay Caused by Hidden Decay
A patient came to us with a cracked e.max ceramic restoration - a thin yet durable ceramic onlay designed to handle significant chewing forces. But even it can fail, and this patient came in for a replacement.
In this case, the restoration didn’t crack by chance. Secondary decay had developed underneath and damaged the tooth structure. A cavity formed under the ceramic, and the restoration fractured while chewing.

The solution required a team approach:
- A prosthodontist carefully removed the old restoration
- A general dentist treated the hidden decay and performed an initial composite build-up
- Instead of the previous restoration, we made a zirconia crown. It offers high strength and excellent aesthetics, fully protects the tooth, and keeps the smile looking natural
- All restorative steps took less than two weeks - fast, comfortable, and pain-free
Results
- The tooth was fully restored and protected from further damage
- The smile looks natural and эстетично
- The patient regained comfort while chewing and confidence in their smile
- This case was handled by prosthodontist Roman Shulga and general dentist Irina Erofeeva to ensure the best function, aesthetics, and long-term result
